Insurance Covered Physical Therapy:
This is a more traditional way to utilize SMART Physical Therapy. You may see a Doctor for a condition for which he or she feels you could benefit from Physical Therapy. The Physician will give you a piece of paper called a referral that you bring to your first visit that directs the Therapist on treatment. The referral you are given can be used at any Physical Therapy office. Your insurance may not require a referral and you can come directly to Physical Therapy without seeing your Doctor. This is called using Direct Access Physical Therapy. If upon evaluation the Therapist feels you need to see a Physician before continuing you will be directed to do so. We treat a wide variety of conditions and patients of all ages.
Direct Access Physical Therapy outside of insurance:
This is a less traditional way to utilize SMART Physical Therapy. People come outdside of insurance coverage when they have a high deductible at the end of the year, when they want to get started in PT right away, or when they do not have insurance coverage. Personal Fitness Appointments:
This is a service for someone who may not have a painful condition but would like to improve their Health and Fitness and would like someone who is medically trained to guide them. Tim McCullough has worked as a Fitness Trainer in the past and has worked with Athletes of all levels and ages. As an Athletic Trainer he has developed injury prevention and training programs for teams of many sports. If you want to improve your performance in a sport but do so with minimal chance of injury this is a good program for you. There will be a thorough initial assessment and screening which will include a medical history and review of old injuries. The cost is a standard hourly rate of 100 dollars for 1 hour initial evaluation and 50 dollars for 30 minute follow up sessions. Group rates for follow up sessions can be organized if friends want to workout in a group session to reduce the cost. Baseball Injury Screenings
Tim McCullough has always been involved in Baseball in some way. In his youth he was a player, in College he was a Student of Coaching under Charlie Roys, and as an adult he was a youth coach at all levels. As an Athletic Trainer and Physical Therapist Tim has taken care of players injuries on a daily basis, provided injury prevention lectures in local communities, and performed injury screenings for pitchers. Tim's Pitching injury prevention interest peaked in 2006 and in 2007 at the National Athletic Trainers Association's annual conventions in which he heard several speakers present the staggering statistics concerning the increase in pitching injuries in recent years. The most influential speakers were Dr. James Andrews and Dr. David Lintner. Tim returned to work and developed an Baseball Injury Prevention Presentation based on the latest research. He started doing presentations to baseball groups in the Community with excellent feedback from Coaches and Parents. He continues to update and present the information to groups in the community.
The Baseball Screening is 1 hour and includes Baseball specific testing, A musculoskeletal exam and exercise prescription to improve the impairments found during the exam. Tennis Injury Prevention and Performance Screenings
Tim McCullough is a Physical Therapist and Certified Athletic Trainer and has performed screenings for High level Tennis Players for many years. Tim worked with the top Junior players in the country performing injury screenings and Physical Therapy with the Junior Tennis Champions Center in College Park, Maryland. He performed yearly screenings of these young athletes witnessing changes in their physical limitations with growth spurts, injuries, and changes in training. He understands how to assist the athlete to train and compete at a high level without putting them at unnecessary risk of acute or overuse injury. Tim's background in Athletic Training and Physical Education at Springfield College gave him the framework to safely progress skill development, strength, and flexibility in people of all athletic levels and ages.
The screening process is just important to an average player who participates for health and general exercise as it is for an Elite Athlete. It will identify impairments in mobility, strength, flexibility and coordination that might put one at risk for injury or affect performance. The screen is based on the USTA's screening procedure but Tim has added a functional movement and a musculoskeletal portion to the screen based on his previous experience.

Golf Fitness and Injury Prevention Screenings
Estimates suggest that 50% of golfers will develop chronic physical problems. Research has also shown that 10-33% of Pro golfers play injured and 50% of them will sustain an injury to prevent participation for 3-6 weeks. In Amateurs the most common injury (nearly 50%) is low back pain and the cause of most injuries in Amateurs is considered to be their mechanics.
At SMART PT we provide the perfect combination of skills to serve the Golf Community. Tim McCullough is a Certified Athletic Trainer and Titleist Performance Institute Certified Golf Fitness Instructor. He has been doing sports screenings for years and has recently started doing Golf Screenings and Golf Injury Presentations at Eastman Golf Links. Tim has extensive training in the treatment of low back pain including Peter O'Sullivan's Cognitive Functional Therapy and motor control approach. This approach looks at how back pain can lead to a change in movement patterns that are maladaptive. This treatment approach is effective in providing strategies to correct these faulty movement patterns which may help the mechanical issues found in Amateur Golf.
The Screening has 3 goals:
1. To identify the impairments of the client that could lead to injury or decreased performance.
2. To generate solutions in the form of exercises and treatment strategies that will address the impairments successfully.
3. To give the client recommendations if the impairments are beyond solving with a simple exercise programs. This may include subsequent exercise progressions, Physical Therapy, or a consult with a Physician.
SMART PT will also work together with your local Golf Professional Pro to make you an injury free and more effective Golfer.
The Screening is comprised of Three parts:
1. The Titleist Performance Institute's Golf Screening
2. A specific Musculoskeletal and advanced movement screening that is performed based on the results of the Golf Screening.
3. Exercise Program will be prescribed based on our findings in the assessment. This program will be made up of videos and handouts to clearly guide the client on how to address their impairments.
